Documents Checklist for Portugal Visa

Mandatory Requirements:

Passport:

Original Passport with validity of minimum three months (from the date of return) and minimum two blank pages for visa stamp.
a) Copy of pages of passport containing personal information and also pages providing important travel information
Please note: 
a) Passports issued beyond 10 years will not be accepted by the Embassy.
b) Indian Passports can only be accepted if their bio data (name, date of birth, place of birth & sex) have not been modified by an “observation”. However, observation regarding the validity of the passport or any observation on the last page is acceptable.
c) Also attach all your old passports (if any)

Shengen Visa Application Form:

Visa application form has to be filled online, once the applicant fill the form,the printout has to be taken out signed by the applicant and attached with the application for submission.

Proof of Employment:

Student 

a) Leave Sanction Letter from school including phone number and address of school 

In case of Minor

a) An NOC by parents , ITR and Bank Statement
b) If one of parents is accompanying the minor then an NOC attested by notary has be to submitted by the other parent 

Self Employed

a) Company Registration Certificate / Certificate of Incorporation 
b) Brief Company Profile 
c) Articles of Memorandum (In case the applicant is MD/Director) 
d) Also attach Proof of Proprietorship / Partnership (In case applicant is Proprietor / Partner in a firm) 
e) Import / Export License (if applicable) 

Employed

a) Proof of employment like Salary Slip or Appointment Letter / NOC from Employer.

Authorisation Letter:

Applicant will have to submit an authorisation letter authorising Bookrumz to submit and collect documents.

Photo Specification:

Two recent passport size colour photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35 mm x 45 mm) 
Please note: Photograph should not be more than 3 months old,scanned/stapled and should not be used in any of the previous visas.

Personal Cover Letter:

Covering-Letter from applicant on business letter head mentioning name, designation,passport number, purpose and duration of visit in brief. The letter should be duly signed by authorised signatory with company stamp and addressed to – The Visa Officer, Embassy of Portugal, New Delhi.
Please Note: Covering-Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.

Flight Tickets :

Ticket Itinerary

Hotel Reservation :

So hotel Confirmation for the entire period of stay as per the tour itinerary 
Please Note Hotel should match the ticket dates and maximum stay should be in Portugal

Medical Certificate/Insurance :

Original Medical Insurance for the stay duration amounting to 30,000 Euros. Please Note : Insurance has to be done by the Authorised Insurance Company.

Financials/Other statements

a) Personal ITR for last 3 years 
b) Also attach Original Bank Statement for last six months with Bank Seal and Sign. Note :Bank statement Should be on A 4 Size Paper. 
c) So attach Copy of Pan Card 
d) Three months salary slip (If Employed).
