Documents Checklist of Poland Visa

Mandatory Requirements:

Passport:

Original Passport with validity of at least 3 months after the intended date of departure from Schengen area and two blank pages for visa stamp. (Documents Checklist of Poland Visa)
a) So copies of the first/last/observation/correction pages 
Please note :
a) So passports issued beyond 10 years will not be acceptable in the Embassy 
b) Also attach all your old passports (if any)
c) So Indian passports with any “observation” will not be acceptable.

Personal Cover Letter: (Documents Checklist of Poland)

Covering-Letter from applicant on business letter head mentioning name, designation,passport number, purpose and duration of visit in brief. So the letter should be duly signed by authorised signatory with company stamp and addressed to – The Visa Officer,Embassy of the Republic of Poland, New Delhi.
Please Note: Covering-Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.

Proof of Employment: (Documents Checklist of Poland)

Self Employed 
a) So Company Registration Certificate / Certificate of Incorporation / Proof of Proprietorship / Partnership Deed 
b) So brief Company Profile 
c) So articles of Memorandum (In case the applicant is MD/Director) 
Employed 
a) So proof of employment like Appointment Letter / NOC from Employer 
Student 
a) So leave Sanction Letter from School 
b) So parents have to provide ITR, Bank statement and NOC letter.

Accommodation Proof:

Hotel confirmation for the entire period of stay in Schengen area and Tour Itinerary 
Additionally Note: Hotel should match the ticket dates and maximum stay should be in Poland.

Flight Reservation:

Confirmed return Air ticket.

Medical Insurance:

Original Medical Insurance for the stay duration amounting to 30,000 Euros. 

Visa Application Form:

Visa application form has to be filled online by the applicant. 
Please Note : Appointment for submission needs to scheduled online after filling the online form.

Photo Dimension:

So two recent passport size photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35 mm x 45 mm) 
Please note: Photograph should not more than 3 months old,scan or staple and should not used in any of the previous visas. 

If Employed

a) Also attach personal ITR for last 3 yrs 
b) Attach three months salary slip 
c) Also attach original Personal Bank Statement for last six months mentioning the Bank’s name, Bank’s Telephone Number clearly with Seal & Signature Note :Bank statement Should be on A 4 Size Paper.
d) Also attach credit Card copy/Foreign Exchange endorsement/Traveller’s Cheque 

If Self Employed 

a) Attach personal & Company ITR for last 3 yrs 
b) Also attach original Personal & Company Bank Statement for last six months mentioning the Bank’s name, Bank’s Telephone Number clearly with Seal & Signature Note :Bank statement Should be on A 4 Size Paper.
c) Also attach Credit Card copy/Foreign Exchange endorsement/Travel
Self Employed 
a) Also attach Company Registration Certificate / Certificate of Incorporation / Proof of Proprietorship / Partnership Deed 
b) Attach Brief Company Profile 
c) Also attach Articles of Memorandum (In case the applicant is MD/Director) 
Employed 
a) Attach proof of employment like Appointment Letter / NOC from Employer 
Student 
a) Also attach leave Sanction Letter from School 
b) Also attach parents have to provide ITR, Bank statement and NOC letterer’s Cheque.

Authorisation Letter:

Applicant will have to submit an authorisation letter authorising Bookrumz to submit and collect documents.
